It contained copies of several missing nursing sheets and an earlier version of the medication record.<\/p>\n<p>The supervisor had preserved them because the case was flagged for internal review.<\/p>\n<p>Laura arranged for the documents to be examined.<\/p>\n<p>When Dr. Price compared the duplicate file with the chart previously supplied, she found that the medication entry had indeed been changed.<\/p>\n<p>The earlier record showed a different administration time.<\/p>\n<p>It also contained a notation that did not appear in the later version.<\/p>\n<p>I asked Laura what the missing notation said.<\/p>\n<p>She read it slowly.<\/p>\n<p>\u201cPhysician notified.
